As a home health RN for BRIO, you will provide home health nursing for patients in their home. You will closely collaborate with the attending physician, patients and their families, and other members of the patient care team for continuity of patient care to help patient resume their independence. Other team members may include physical therapy, occupational therapy and home health aide services.
Role:
Assess and reassess all physical, environmental and emotional factors to determine home health needs for patients in the home setting.
Work in cooperation with the family/caregiver and IDT to meet the emotional needs of the patient/family/caregiver.
Actively participate in Interdisciplinary Team (IDT) conferences.
Requirements:

• 
  Positive and friendly nurse.
• 
  Current and unencumbered license to practice as a Registered Nurse in Utah.
• 
  Minimum of one (1) year of home health experience as a registered nurse (RN).
• 
  Current CPR certification.
• 
  Valid Driver's License with reliable transportation and agency required liability insurance to complete home visits.
• 
  Must be open minded and teachable.
• 
  Must pass background check.
