Physical Therapist Location: West Jordan, UT Agency: Mindlance Health Pay: $1,966 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 1/26/2026 About the Position ID: 75002116 Shift: 9H Days 8a-530p; 1 weekend per month Description: Unit Notes:- Hiring Process: Profile Review: Profile Review with MedSol Clinical, Interview with Unit manager at facility- Radius:: 60 MILES, DL must be submitted- Week of orientation: Please ensure clinicians are in state for week of orientation even if first day modules are General Certification Requirements (BLS, etc.)BLS- Other Certs Required for all PT:\"Physical Therapist license from a participating state in the National Licensure Compact- Dry Needling Certification: ONLY REQUIRED If performing the Dry Needling function: Dry Needling Certification orEducation\"- Common diagnosis / Types of patients\"Providing care to patients with neuromuscular, orthopedicand other physically-related disorders requiring strengthening, modalities, manual therapy,neuromuscular re-education and training in activities in daily living under the supervision of a licensedphysical therapist. Participates in professional responsibilities, including documentation, program development, teaching, and enhancement of clinical skills\" of therapeutic interventions including, but not limited to, ADL retraining; patient education in home safety; adaptive equipment and compensation techniques; coordination, strengthening and therapeutic exercise programs, manual therapy, therapeutic activities, aquatic therapy and neuromuscular re-education per patient needs- Experience or 'Must have' skills & common job duties\"Previous work experience providing direct patient care in outpatient, inpatient or inpatient rehab unitpreferred. Prior use of electronic medical records (EMR), preferred\"- Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?)\"This position also may require you to work standard hours/shifts, as well as flexible hours before and after standard hours/shifts, and overtime or in excess of 40 hours in a work week.
